You are being asked to participate in a research study. The box below highlights key information about this research for you to consider when deciding on whether or not to participate. Carefully consider this information and the more detailed information provided below the box. Please ask questions about any of the information you do not understand before you decide whether to participate.

Key Information for You to Consider
· Voluntary Consent. You are being asked to volunteer for a research study.  It is up to you whether you choose to participate or not.  There will be no penalty or loss of benefits to which you are otherwise entitled if you choose not to participate or discontinue participation.

· Purpose. The purpose of the mixed method study is to identify the specific motivational components of graduate level students in a healthcare EdD program at a private university.

Duration. You will complete an online survey that will take approximately 15-20 minutes and a virtual interview that will take approximately 45-60 minutes.

·  Procedures and Activities. You will be asked to fill out the Academic Motivation Scale (AMS-C-28) survey that will be hosted through the SmartSurvey website. Along with the survey, participants will be asked to complete a semi-structured interview that will be conducted virtually through Ring Central. The interview will take approximately 45 -60 minutes to complete. During the interview, you will be asked a series of questions related to your motivations for pursuing an EdD degree and the challenges you have encountered. The interview will be recorded (audio only) for transcription purposes. You will be asked to turn off your camera prior to the interview beginning. The recording will ensure that the findings are accurately attributed to your specific experiences within the university context.

·  Risks. Risk will be no more than what occurs in everyday life.

·  Benefits. No direct benefit, but the researchers hope to expand the existing body of knowledge by focusing on a group with unique educational and professional challenges. The insights garnered through this study will help to inform educators and students of motivational influences that may affect their future academic achievements.

·  Alternatives. Your participation is voluntary, and the only alternative is to not participate.

Why is this research being done?

The purpose of the mixed method study is to identify the specific motivational components of graduate level students in a healthcare EdD program at a private university. Specific motivational influences of graduate students have not been fully identified. Given the theoretical frameworks of the Self-Determination Theory and the Self-Efficacy Theory, this study aims to explore how intrinsic and extrinsic motivators affect students’ retention rates.

What Will Happen in This Research Study

Online Survey

You are being asked to complete a 28-question anonymous online survey. The survey should take approximately 15-20 minutes to complete. The survey will be hosted through the SmartSurvey website.

Semi-Structured Interview

After completing the survey, you will be instructed to review a schedule for the semi-structured interview, which you can access through the Sign-Up Genius link presented at the end of the survey. The virtual interview will take place through Ring Central and will take approximately 45-60 minutes to complete. During the interview, you will be asked open-ended questions to explore your motivations for continuing in the program. Follow-up questions will be asked to clarify the specific support systems that may have affected your choice to persist.

The interview will be recorded for audio transcription only. You will be asked to turn off your camera prior to the interview beginning. The recording will ensure that the findings are accurately attributed to your specific experiences within the university context.

The ways we will protect your privacy and confidentiality are described in a separate section later in this form.

You will be one of approximately 100 subjects who will be asked to be in the study.

Risks and Discomforts

Some potential risks with participation include responding to questions about your personal experiences, educational background, or motivations. The time it takes to participate in the study may also take time away from personal activities, but this time commitment will be minimal. Participation in this study is entirely voluntary, and participants will be allowed to withdraw from study at any time without penalty or negative consequences. You may choose not to answer any questions about which they feel uncomfortable. The decision to participate or not will not affect your standing in the EdD program or any other university-related activities.

Potential Benefits

You will receive no direct benefit from being in this study, but the researchers hope to expand the existing body of knowledge by focusing on a group with unique educational and professional challenges. The insights garnered through this study will help to inform educators and students of motivational influences that may affect their future academic achievements.

Confidentiality

We must use information that shows your identity to do this research. Information already collected about you will remain in the study record even if you later withdraw. We will store your information in ways we think are secure. We will store paper files in locked filing cabinets. We will store electronic files in computer systems with password protection and encryption. However, we cannot guarantee complete confidentiality.

If you agree to be in the study and sign this form, we will share information and biological samples that may show your identity with the following groups of people:
